This page provides links to the Forms you will need to open and operate a Telebet Account with UNiTAB.

Note: To comply with federal legislative requirements you need to have your identification verified before your account becomes unrestricted. This can be done through Australia Post, by an Operator at a TAB Retail Outlet, or by one of the Acceptable Referees listed on the Telebet Account Application and Verification form. See below for information on which Form to use.

Forms are in PDF format and will open in a new page of your browser. Parts of some of the Forms can be filled in online prior to printing and presenting to an Australia Post outlet or TAB Retail Outlet, or forwarding to Telebet.

To have your identification verified at an Australia Post outlet, take the Australia Post Telebet Identity Verification Form to a participating Australia Post outlet. Australia Post will forward the form to Telebet.

Alternatively, you can apply for an account and have your identification verified by completing the Telebet Account Application Form at any TAB Retail Outlet in Qld, South Australia or Northern Territory. The TAB outlet will forward the form to Telebet.

If you are unable to access an Australia Post or TAB outlet, you can have your identification verified by an Acceptable Referee. Take the Telebet Account Application & Identification Form to one of the Acceptable Referees noted on Page 3 of the form and then forward the completed form to Telebet at: PO Box 248 Albion 4010 or fax to 07 38623095.

Once your unrestricted Account is established, you can (if you wish) organise to have a credit card activated for future deposits into your Account. To do this, complete the Credit Card Activation Form and forward to Telebet at: PO Box 248 Albion 4010 or fax to 07 38623095.
